Documents Required for Claims (In Case of Advance Payment)

• Claim reimbursement request form (can be downloaded at https://dsa.su.ac.th)
• Original payment receipt
• Original medical certificate stating the reason for treatment
• Medical treatment records (only if the insurance company requests them)
• Copy of national ID card (certified true copy)
• Copy of the insured person’s bank account passbook (for reimbursement transfer)

Insurance policy details can be viewed, and service centers can be searched via the Muang Thai Friend App.

Coverage

• Death due to accident: 250,000 THB per person
• Medical expenses from accident: 25,000 THB per visit
• Funeral and funeral management expenses in case of death from illness: 35,000 THB per person
• Other coverage* can be found through the university’s public relations channels
